Eligibility
- Programs where all ages are eligible
- Refugees
- Muscular Dystrophy
- Must be a registered client of Muscular Dystrophy Canada
- Be member of a Muscular Dystrophy Canada Chapter
- Must have a confirmed diagnosis of neuromuscular disorder under the Muscular Dystrophy umbrella, signed by a physician or a health professional
- Canadian citizen, landed immigrant, or refugee resident in Canada
What Devices are Covered?
- Access equipment, including: door openers, ramp, platform lift, stair lift
- Bathroom aids, including: commode/toileting device, bathing/shower device, other bathroom aids
- Beds and mattresses, including: hospital bed (manual and electric), mattresses
- Lifts, including: hydraulic lifting device and sling, transfer aid, track lift, chair lift
- Communication devices
- Mobility devices, including: power wheelchair, power tilt/recline seating, manual wheelchair, rollator/walker/walking device, scooter
What Services are Available?
- Equipment costing less than $200 will not be eligible for funding
- Repair and modifications
- Batteries and mattress replacement
- Short term rentals of equipment
Procedure for Getting Access
- Completion of the application form
- Requires a signed equipment prescription from the appropriate licensed health care professional such as: an occupational therapist, physiotherapist, respiratory therapist, orthotist, physiatrist, registered nurse or podiatrist
- Quotes from 2 different equipment vendors
- Approval or denial letter from insurance company and other funders approached (if applicable)
