Eligibility
- Programs where all ages are eligible
- Multiple Sclerosis
- Diagnosis of MS from physician or MS Clinic
- Permanent resident of Alberta or the Northwest Territories or reside within the Lloydminster & District Chapter region
- You have NOT already purchased the equipment and are not going to do so before your application is reviewed (grants are not retroactive and cannot be put towards equipment that has already been ordered or purchased)
- You are ineligible for sufficient funding from other funding sources (e.g. Alberta Aids to Daily Living, Assured Income for the Severely Handicapped health and personal benefits, Disability Related Employment Supports, Residential Accessibility Modification Program (RAMP), health care insurance, etc.)
What Devices are Covered?
- Bath aids, including: bath lifts, bath chairs, bath bench
- Bracing & supports, including: orthotics, leg braces, splints
- Broda chairs
- Computer with adaptations
- Home accessibility equipment, including: porch and stair lifts, automatic door openers, ramps
- Electric hospital beds and mattresses
- Kitchen aids, including: auto feeders, adapted utensils
- Lift chairs and systems, including: ceiling tracks
- Mobility aids, including: scooters, walkers, wheelchairs
- MS-related eyeglasses
- Toileting aids, including: commodes, raised toilet seats, toilet safety frames
- Transferring aids, including: bets, boards, grab bars, poles
- Other types of equipment is considered with regards to the equipment's direct, positive impact on the applicant’s quality of life and MS symptoms
What Services are Available?
- Equipment funding (maximum annual amount is $1,000 per applicant per the calendar year; however, more can be requested with submission of income tax assessments and demonstrating extraordinary financial needs)
- Equipment repairs and other services
Procedure for Getting Access
- Submission of application form
- Further documentation may be requested, including: the type of request, the submitted quotes, available resources, and financial need
- Completion of the assessment form or letter of support from an appropriate provider (e.g. occupational therapist, physiotherapist, physician)
- Funding must be used within 3 months of approval
